State and Territory Health Departments

Following are links to relevant policies and patient identification programs being undertaken by State and Territory health departments.


ACT

The 5 Step Correct Patient, Correct Site, Correct Procedure policy (PDF 73KB) was released by ACT Health in 2005.


NSW

Correct Patient / Patient Identification is a state-wide initiative of the NSW Health Safety and Quality Branch. The Correct Patient, Correct Procedure and Correct Site policy (PDF 105KB) was released in 2004. In addition, Safety Toolkits have been developed for specialist practitioners and distributed to radiologists, radiographers and oral health facilities throughout NSW to assist in compliance with the Correct Patient, Correct Procedure and Correct Site policy.

The Australian Resource Centre for Healthcare Innovations is supported by NSW Health as part of their commitment to knowledge sharing. The South Eastern Sydney Illawarra Area Health Service entered the ARCHI patient identification project in the Baxter 2006 NSW Health Awards for identification of patients receiving chemotherapy.


Queensland

Ensuring Intended Surgery is an initiative of the Queensland Health Patient Safety Centre (PSC). The Ensuring Intended Surgery (EIS) Policy (PDF 218KB) was released in 2005.


Victoria

The Correct Side, Correct Site Surgery Guidelines (PDF 189KB) developed by the Royal Australasian College of Surgeons (RACS), included a representative from the Victorian Surgical Consultative Council.


South Australia

The Safety and Quality Unit provides information and resources for health professionals and consumers. The Correct Patient, Correct Site Correct Procedure policy was released in 2006. Alerts and Advisories provided safety notices for Wrong Patient, Wrong Site Radiological Incident Alert (PDF 51KB) and Patient Identification (PDF 45KB).


Western Australia

The Office of Safety and Quality in Healthcare has a variety of information and resources on patient safety and quality. In November 2006, the Correct Patient, Correct Site and Correct Procedure Policy and Guidelines for WA Health Services were released.
