In May 2008, a research team, led by the Australian Patient Safety Foundation (APSF) and including Communio, University of South Australia (UniSA) and the Centre for Health Informatics (CHI), was engaged by the Commission to conduct a project to improve learnings from patient safety incidents in the areas of clinical handover and patient identification.
The study explored a method of using machine learning technology to automatically identify critical safety incidents and engaged the use of existing incident monitoring and analysis tools - AIMS classification, Human Factors and Root Cause Analyses - to investigate and identify key safety factors.
