The National Medication Safety Breakthrough Collaborative (NMSBC) was a key initiative of the Council to reduce patient harm from medication use. While enhancing medication safety is an extremely complex area of work, the Collaborative demonstrated how significant improvements can be made through effective multi-disciplinary collaboration and small scale testing of changes at the facility, state and national levels.

This is the first in the series of handbooks and covers information on the national and international support and advice networks for the collaborative. It covers the experiences of the project team who undertook this exciting initiative.

This document is designed to help you and your organisation improve medication processes and safety by showcasing the experiences of NMSBC teams. The document illustrates the work that has been completed by each team, highlighting their contribution to service improvement activities related to the safe use of medicines in the hospital and community settings. The summaries are complemented by the improvement guide, which lists and shows examples of the successful innovations and tools created by the teams.
