Consumer Information sheet: Mistakes can happen with your medicines

The A4 consumer information leaflet provides helpful tips on how consumers can be involved in preventing medicine mistakes when going in and coming out of hospital, changing wards or seeing different health professionals. The main message is to keep track of all the medicines they are using with the use of a Medicines List and showing this information when they visit a health professional or are admitted to hospital.

Consumer Information wallet: Important information about your medicines

The consumer information wallet is an A5-sized consumer information wallet which provides helpful tips on how patients can be involved in preventing medicine mistakes when going in and coming out of hospital, changing wards or seeing different health professionals. The intent is to is help patients keep track of their medicines through the use of a Medicines List, and to show this information when they visit a health professional or upon admission to hospital.

The wallets are designed to be provided to patients on discharge from hospital, and can hold the discharge medicines list and other medicines information. 

Hospitals can have them printed from the attached files.
