The Australian Commission on Safety and Quality in Health Care reports annually on the state of healthcare safety and quality in Australia.

The Windows into Safety and Quality in Health Care 2009 provides windows onto a range of safety and quality issues and continues to report on sentinel events. It offers safety and quality insights in a number of settings and from various perspectives.

This paper provides information, resources and examples of a patient-centred approach to care delivery.

This report, Windows into Safety and Quality in Health Care 2011, builds upon the previous years’ Windows reports and offers perspectives on a range of healthcare safety and quality matters in a number of settings. In part, it provides a review of the activities of the past five years. It also anticipates some of the emerging issues and challenges that the Commission may address, including the culture of health care, the importance of patient-centred care, and safety and quality in primary care.

This report has been prepared by the Commission to present findings from the review of the Standard. It is intended for the people and organisations that have a role to play, and an interest, in open disclosure and aims to:

• present findings from a review of the Standard drawing on current research and evidence of, and experience with, disclosure
• identify where the Standard does and does not reflect current evidence and practice
• recommend changes to the Standard.
