The 2008 Australian Charter of Healthcare Rights was reviewed in 2 phases. This report details the first phase of the revision process. 

The end-of-life care audit tool was developed to give hospitals the ability to compile data associated with the delivery of end-of-life care in a manageable and uniform way.
