The National Residential Medication Chart User Guide for Pharmacists is intended for pharmacists who provide services to residential care facilities (RCFs) and who are authorised to supply and claim for medicines under the Pharmaceutical Benefits Scheme (PBS) or Repatriation Pharmaceutical Benefits Scheme (RPBS).

The Pharmaceutical Benefits Scheme Hospital Medication Chart (PBS HMC) is a national standardised medication chart that allows the prescribing, administration, claiming and supply of PBS and non-PBS medicines directly from the chart without the need for a separate paper prescription.

The National Medication Management Plan (NMMP) is a standardised form to improve the accuracy and completeness of documented information to support continuity of medication management and medication reconciliation during transitions of care. The user guide has been developed with the support of the Health Services Medication Expert Advisory Group incorporating a number of elements originally devised by WA Health.
