A comprehensive care plan is a document describing agreed goals of care, and outlining planned medical, nursing, midwifery and allied health activities for a patient.

The Pharmaceutical Benefits Scheme Hospital Medication Chart (PBS HMC) enables the prescribing, administering, supply and claiming of eligible PBS and non-PBS medicines without the need to issue a separate prescription.

Clostridioides difficile (C. difficile) is an anaerobic, spore-forming, gram-positive bacillus typically associated with gastrointestinal disease. Transmission of C. difficile occurs by ingestion of spores either through person-to-person contact, animal-to-person contact or environment-to-person contact.

In 2019, Professor Lynne Emmerton led the review of name similarity for:

• Monoclonal antibodies (MABs) (commonly ending in the suffix ‘mab’)
• Tyrosine kinase (factor) inhibitors (TKIs) (commonly ending in the suffix ‘nib’).

This included the theoretical risk of confusion within these classes of medicines in clinical practice, including the similarity in clinical indication, with a view to producing a specialised list of medicines. The results of this review can be found in this publication.

This report by the University of Sydney School of Pharmacy informed the development of the National standard for labelling dispensed medicines. The report summarises qualitative user testing of prototype labels with consumers in Australia. Subsequent stakeholder consultations were undertaken to finalise the Standard.

This alert provides more information in regard to concentrated intravenous potassium chloride. Inappropriate injection of undiluted intravenous potassium chloride has been implicated in fatalities in Australia and overseas.

This report draws on data from a wide range of sources, and includes information about key advances in safety and quality in Australia; prevalence of common safety risks to patients; action taken to identify and drive the delivery of appropriate care; and the Commission’s approach to supporting value based healthcare.

The analysis and data provided in this report are limited to public hospitals. The Commission is currently working with the private hospital sector to ensure the specified safety and quality indicators can be included in future reporting.
