This document outlines some changes made to MedicineInsight as part of the transition from NPS MedicineWise to the Commission, along with the actions required by reconsenting practices and participating general practitioners to continue involvement in the program.

GPs at participating MedicineInsight practices who would like the practice clinical activity reports to identify them by name must complete this consent form and return it to the Commission via medicineinsight@safetyandquality.gov.au.
