Toolkit
The End-of-life care audit toolkit contains a series of tools for undertaking an end-of-life care audit and staff survey. It is designed to help health service organisations to examine and improve the quality of their delivery of end-of-life care.
Development process
To support hospitals measure and identify opportunities for improvement in end-of-life care, the Commission partnered with the Australian National University and 10 Australian hospitals, to develop and test the end-of-life care audit and clinician survey.
Following the completion of two stages of testing, the tools were brought together in an End-of-life Care Audit Toolkit which includes:
- End-of-life care audit tool
- Clinician survey tool
- Hospital demographic information survey tool
- Data dictionary
- Analysis advice with a plan and table templates
- Presentation template.

Publication, report or update
The end-of-life care audit tool was developed to give hospitals the ability to compile data associated with the delivery of end-of-life care in a manageable and uniform way. Having the capacity to collect and analyse this data allows hospitals to review how care is being delivered and identify opportunities for better alignment with the Consensus Statement, and improvement of end-of-life care in general.
The findings of the pilot of the initial end-of-life care audit and survey tools is also available.
