Transferring all or part of a patient’s care between healthcare providers or locations is a high risk situation, and there is an increased risk of communication errors at these times. Effective clinical handover, which is structured and standardised, can reduce communication errors and improve patient safety.

The Commission has developed the Communicating for Safety resource portal which provides guidance on the strategies and skills needed to support effective communication at clinical handover. The portal highlights why communication is important at transfers and links to useful resources.
