The webinar will explore how health services can provide better, more accessible care to people with disability through the collection of self-reported patient information on disability.

Hear about the Victorian Disability Liaison Officer (DLO) Program Disability Identifier Project. The Disability Identifier has been co-designed with people with disability, clinicians and technical experts and is a way for people with disability and their carers to tell healthcare staff about any specific needs prior to their visit or stay in hospital.

Joanne Rowe, Project Lead will discuss how the initiative works, system integration, involvement of patients and families and how to adopt a similar initiative in your health service. 

Participants will also have the opportunity to take part in a live Q&A session with the presenters and a panel of speakers.
