This tool has been developed to assist health service organisations implement open disclosure in accordance with the Australian Open Disclosure Framework.
This tool is designed to assist health service organisations ascertain how well they meet the necessary requirements to implement open disclosure and highlight areas for improvement.
The template should be adapted to suit local requirements and applied in accordance with the Australian Open Disclosure Framework.
