This resource is a survey template that enables healthcare providers to provide feedback about the open disclosure process.
This survey has been developed to enable feedback from healthcare providers about the open disclosure process.
The aim of this survey is to improve the open disclosure experience for the person who was unintentionally harmed, their support people and healthcare providers.
The template should be adapted to suit local requirements and applied in accordance with the Australian Open Disclosure Framework.
