CARAlert Isolate Referral Form
Isolate Referral Forms must not be forwarded to the Commission. A completed Isolate Referral Form must be sent with a suspected CAR isolate from an originating laboratory to the appropriate confirming laboratory to enable submission to CARAlert via the web portal.
